An Audit Report during the UIC Interoperability Study November 2005 concluded

“Overall the audit revealed that there is a good level of understanding of the importance of Health & Safety amongst all the persons involved in the UIC Trials with this matter being considered as being of paramount importance.

Generally the standards being displayed on the site with regards to operational safety can be regarded as demonstrating best industry practice. This in turn demonstrated that all the significant risks that were identified in the method statement were being adequately controlled and managed.”

Note worthy good practices include:

The comprehensive off site pre-start site briefings that were carried out by the DMQA COSS following his clarification of documentation irregularities

Good communications between all members of the team

The COSS intervening when he believed that safety could be compromised by distracting the lookout despite the works taking place in a possession

Mr Baker challenging the auditor over who was his Sentinel Sponsor.
